Plan Notes

  • This plan pairs with a Health Savings Account, and Modera contributes to that account on your behalf. As an added benefit, Modera will contribute the difference between the employee-only PPO Plan and High-Deductible Plan premium (currently $206.69) to your account each month.
  • The High-Deductible / HSA Plan may be a fit if you want Modera’s contribution to an HSA that you own and keep, cover dependents and want a lower premium, or want to save tax-free for future healthcare expenses, including in retirement.
  • The family deductible and out-of-pocket maximum are higher on the High-Deductible / HSA Plan.
