Plan Notes

  • The vision plan uses the VSP Choice Network, whose providers offer you higher benefits at a lower cost. Consider using an in-network provider for the most bang for your buck when you need services!
  • Contact lenses: medically necessary contacts are covered in full after the $25 copay; elective contacts have an allowance of up to $150; the fitting and evaluation copay is no more than $60.
  • Employees can elect dental and/or vision regardless of their medical enrollment status.
